Radiation therapy treats a particular part of the body and not the whole body. Endometrial cancer, if it recurs, is most likely to come back at the top of the vagina. In high-risk cases, recurrence is 50% local (in the pelvis) and 50% systemic (anywhere in the body).

Pelvic radiation is used to decrease the risk of local recurrence (pelvis and vagina). Women who have a cancer that is deeply invasive into the muscle or high grade cancer may consider radiation therapy to decrease the risk of local recurrence.
